Note Taker Agreement for Students

Requesting a Note Taker

  1. Students must be approved by a Program Specialist for note taking services.
  2. Student’s must  provide the Note taking Coordinator with a copy of their class schedule indicating which classes they need  note taking services for.
  3. Students are responsible for securing their own note taker for each class. Student’s should pick up Note Taker Request Form from Accessibility Services or download online at: http://as2.unm.edu.
  4. If you need assistance in securing a note taker within your class please contact Accessibility Services at 277-3506.

 

Once a Note Taker is secured

  1. The note taker will be given the student’s e-mail and phone number so they may contact the student outside of the classroom to ensure confidentiality.

  1. The student will make the arrangements with the Note taker for receiving the class notes.
  2. If a student requests to remain anonymous, the student will be required to pick up their notes from our office in Mesa Vista Hall.
  3. It is the student’s responsibility to supply paper to the note taker. If the note taker is enrolled in the class, then either the Note taker or the student may pick up NCR (carbonless) paper from our office.
  4. It is the student’s responsibility to speak with the note taker if there is a problem with the notes. If the problem is not resolved, then ask for assistance from the note taking coordinator. If the note taker is not showing up to class, please notify the coordinator immediately so a new note taker can be provided.
  5. Students using the services of a note taker should attend all class sessions. Note takers are not a substitute for attendance. Note takers do not supply notes if you are not attending class.
  6. If a student is unable to attend class due to a disability related reason or emergency, or for an extended period of time, it is mandatory that you contact the note-taking coordinator and the note taker. In some instances or special circumstances, it will be approved for the note taker to attend class. It is the student’s responsibility to get approval from their assigned Program Specialist if a special circumstance arises.
  7. Students are required to pick up notes in a timely manner (weekly). If there becomes an issue with student’s not picking up notes, your services will be suspended and you will be required to meet with a Program Specialist to continue your services.
